Case Manager Social Adults Care


Job description


Hanna Z Adult Daycare is seeking an Adult Day Care Center Case Manager responsible for overseeing the care and support services for clients attending the center. The Case Manager will also work closely with clients, their families, and the team to develop and implement individualized care plans, ensuring each participant’s physical, emotional, and social well-being.


  • Conduct initial and ongoing assessments of clients’ needs, strengths, and preferences.
  • Collaborate with clients, families, and healthcare professionals to develop comprehensive care plans.
  • Coordinate and monitor the delivery of services to ensure they align with the individualized care plans.
  • Facilitate communication among the team, including healthcare providers, social workers, and therapists.
  • Ensure clients receive appropriate services and resources to meet their unique requirements.
  • Regularly assess clients’ progress and adjust care plans as needed.
  • Monitor and evaluate the effectiveness of services provided, making recommendations for improvement.
  • Maintain accurate and up-to-date client records, documenting assessments, care plans, and client condition changes.
  • Ensure compliance with regulatory standards and documentation requirements.
  • Provide education and support to clients and their families regarding available resources, services, and community programs.


  • High School Diploma or equivalent required.
  • Previous experience in case management, preferably in a healthcare or social services setting.
  • Knowledge of aging, disability, and mental health issues.
  • Proficient in Microsoft Office (Outlook, Word, Excel).
  • Must be organized and detail-oriented with excellent listening, written, and oral communication skills.
  • Self-motivated, able to work independently and with teams, multitask, and make sound decisions when needed.
  • Bilingual Spanish/English (other languages are a plus!)
