Valid New York State CNA certification or LPN/RN Licence and Registration, Valid photo ID, Social Security Card, resume, two reference letters, physical exam (within last year), two step PPD OR Quantiferon lab reports (within last year), proof of MMR vaccination (two doses) OR MMR Titers lab reports, COVID-19 vaccine record (2 doses Pfizer/Moderna or 1 dose Janssen), voided check or direct deposit enrollment form. LPN/RN: educational degree & BLS
